Please be sure to bring your insurance card and any other applicable insurance information to each appointment. Also, be prepared to pay your insurance plan co-payment at the time of your appointment as required by your insurance company. Remember that each patient or family member requires a separate appointment and co-payment. If you are not prepared to pay your co-payment, you may be asked to reschedule your appointment.
As a courtesy, our billing office submits most insurance claims. If you have specific questions about what your insurance plan does or does not cover, please contact your insurance company.
